Researchers at Hunter College and Rutgers University found that oleocanthal ruptured the lysosomes of breast, prostate, and pancreatic cancer cells, triggering rapid death. Healthy cells treated with the same concentrations simply paused their growth temporarily.
Cancer cells have larger, more fragile lysosomes. Oleocanthal exploits this structural vulnerability. Epidemiological data supports the pattern: higher olive oil consumption is associated with lower risk of breast cancer and digestive system cancers across 19 observational studies.

In a clinical trial, 60 women with primary dysmenorrhoea took 25 ml of olive oil daily for two months. Pain scores dropped from 6.6 to 1.1 on a 10-point scale. The ibuprofen comparison group only reached 3.8.
The evidence extends to menopause (significant quality-of-life improvements in 60 postmenopausal women over 12 weeks, including reduced hot flashes and better sleep) and PCOS (improved insulin resistance in a 10-week trial with 72 women).

But the medicine only works at the right dose.
Every study above used olive oils with meaningful polyphenol concentrations. The benefits are dose-dependent. Most supermarket EVOOs contain 50 to 150 mg/kg of polyphenols. The EFSA health claim requires 250 mg/kg minimum. The clinical trials that produced the strongest results used oils above this threshold.
Polyphenols also degrade with oxygen, light, heat, and time. An oil at 500 mg/kg at pressing can lose half its potency in months in a clear glass bottle.
